How do I start a Data Use Checkup for the first time?

I want to request advanced access to email from the Permissions and Features page. When I click the button to make the request I receive the message "You must complete data use checkup certification before your app can go live. Learn more about our data use checkup." We are a pre-launch start-up wanting to use Facebook Login. How do I start a Data Use Checkup for the first time?

@crispin, No. I have not received an answer. And when I saw the notification for your message I was optimistic ... only to find that I'm not the only one with this problem.
If you find a solution, please share ... and I will do the same.
