My business case is to receive and respond to messages sent to a Facebook and Instagram account through a webhook.

On Facebook, it works perfectly, but when I set it to Instagram (even with the Test webhook button working), it does not receive the direct messages sent to the account.

Are the instagram_basic permission required?

We do not need to analyze detailed user information, nor do we need to log in with Facebook or Instagram. Just obtaining the message and a way to identify by ID is enough.
